Population Pharmacokinetic Model for Vancomycin Used in Open Heart Surgery: Model-Based Evaluation of Standard Dosing Regimens
Antimicrobial Agents and Chemotherapy · 10.1128/AAC.00088-18
Structure
A two-compartment model, driven by creatinine clearance, serum albumin and weight.
Study population
Surgery patients — Middle East. Fitted to 28 patients and 168 concentration observations.
| Covariate | As reported |
|---|---|
| Age | 51.7 ± 15.9 yr |
| Weight | 79.6 ± 17 kg |
| BMI | 29.8 ± 5.6 kg/m² |
| CrCl | 83.5 ± 29.3 mL/min |
| SCr | 0.87 ± 0.25 mg/dL |
Reported as the source publication gives them — mean ± SD, or median with range or interquartile range.
Equations
The model's structural parameters as Chorus implements them.

CL = 6.13 \cdot \left(\frac{CL_{CR}}{83.5}\right)^{0.514} \cdot \left(\frac{Albumin}{35.5}\right)^{0.854}
V_1 = 40 \cdot \left(\frac{Weight}{79.6}\right)^{0.466}
Q = 0.22
V_2 = 3.88
Implementation notes
Open-heart surgery prophylaxis model. The published CL includes a serum-albumin term that Chorus does not collect; albumin is fixed to the population mean (35.5 g/L), so that term has no effect.
