Brooks et al. 2026 (young adult)
Leveraging real-world data to create vancomycin population pharmacokinetic models in younger adults and patients with low serum creatinine
Journal of the American Pharmacists Association Pharmacotherapy · 10.1016/j.japhar.2025.100026
Structure
A two-compartment model, driven by weight, age, sex and serum creatinine.
Study population
General patients — United States and North America. Fitted to 6,080 patients and 19,071 concentration observations.
| Covariate | As reported |
|---|---|
| Age | 30 (18–35) yr |
| Weight | 78 (39–210) kg |
| BMI | 27 (15–69) kg/m² |
| CrCl | 150 (32–470) mL/min |
| SCr | 0.8 (0.28–3.3) mg/dL |
Reported as the source publication gives them — mean ± SD, or median with range or interquartile range.
Equations
The model's structural parameters as Chorus implements them.

CL = 4.2622 \cdot \left(\frac{FFM}{55}\right)^{0.2742} \cdot \left(\frac{eGFR_{abs}}{100}\right)^{0.9452}
eGFR_{abs} = eGFR_{CKD-EPI} \cdot \frac{BSA_{DuBois}}{1.73}
V_1 = 40.7349 \cdot \frac{FFM}{55}
V_2 = 48.7348 \cdot \frac{FFM}{55}
Q = 2.9295 \cdot \left(\frac{FFM}{55}\right)^{0.2742}
Implementation notes
Young adult model (18-35). Uses Janmahasatian FFM normalized to 55 kg (not the 50 kg printed in Table 2 — author-confirmed erratum) and CKD-EPI 2021 eGFR de-indexed to absolute mL/min via Du Bois BSA.
