A new population pharmacokinetic model for vancomycin in patients with variable renal function: Therapeutic drug monitoring based on extended covariate model using CKD-EPI estimation
Journal of Clinical Pharmacy and Therapeutics · 10.1111/jcpt.12995
Structure
A two-compartment model, driven by weight, serum creatinine and age.
Study population
General and Mixed patients — Korea and Asia. Fitted to 99 patients and 328 concentration observations.
| Covariate | As reported |
|---|---|
| Age | 64.8 ± 12.6 yr |
| Weight | 59.7 ± 11 kg |
| BMI | 22.3 ± 3.93 kg/m² |
| SCr | 1.7 ± 2.04 mg/dL |
Reported as the source publication gives them — mean ± SD, or median with range or interquartile range.
Equations
The model's structural parameters as Chorus implements them.

CL = 2.21 \cdot \left( \frac{0.921}{eGFR_{base}} + \frac{eGFR(t)}{68.4} \right)
eGFR = eGFR_{CKD-EPI} \cdot \frac{BSA_{DuBois}}{1.73}
V_1 = 32.6
V_2 = 45.8
Q = 3.06 \cdot \left(\frac{Weight}{60}\right)^{0.75}
Implementation notes
Extended covariate model for time-varying renal function, using CKD-EPI eGFR de-indexed to absolute mL/min via Du Bois BSA. The population median denominator is reconstructed from the paper's renal function categories, as the paper reports only a mean.
