Therapeutic Drug Monitoring · 10.1097/FTD.0000000000001303
Structure
A two-compartment model, driven by weight, serum creatinine and age.
Study population
General patients — Canada and North America. Fitted to 73 patients and 525 concentration observations.
| Covariate | As reported |
|---|---|
| Age | 56.3 ± 17.5 yr |
| Weight | 81.5 ± 23.4 kg |
| BMI | 28.1 ± 7.1 kg/m² |
| CrCl | 97.3 ± 41.6 mL/min |
| SCr | 1.21 ± 0.51 mg/dL |
Reported as the source publication gives them — mean ± SD, or median with range or interquartile range.
Equations
The model's structural parameters as Chorus implements them.

CL = 3.96 \cdot \left( \frac{0.00921}{eGFR_{base}} + \frac{eGFR(t)}{eGFR_{median,patient}} \right)
eGFR_{median,patient} = \mathrm{median}\left(eGFR_{CKD-EPI}\right) \text{ over the patient's measured } SCr
V_1 = 39.9
V_2 = 66.4
Q = 5.17 \cdot \left(\frac{Weight}{60}\right)^{0.75}
Implementation notes
Re-estimated Kim DJ 2019 model for osteoarticular infections, using indexed CKD-EPI 2021. The renal term is normalized to the patient's own median eGFR, so clearance tracks change from their typical renal function rather than differences between patients.
