Toward Model-Based Informed Precision Dosing of Vancomycin in Hematologic Cancer Patients: A First Step
Clinical Pharmacokinetics · 10.1007/s40262-023-01329-0
Structure
A two-compartment model, driven by ideal body weight and creatinine clearance.
Study population
Hematologic malignancy and Transplant patients — Canada and North America. Fitted to 60 patients and 240 concentration observations.
| Covariate | As reported |
|---|---|
| Age | 56 (20–78) yr |
| Weight | 85.1 (43–154) kg |
| BMI | 28.8 (17.7–42.2) kg/m² |
| CrCl | 100 (63–142) mL/min |
| SCr | 0.74 (0.35–1.24) mg/dL |
Reported as the source publication gives them — mean ± SD, or median with range or interquartile range.
Equations
The model's structural parameters as Chorus implements them.

CL = 5.63 \cdot \left(\frac{IBW}{63.0}\right)^{0.75} \cdot \left(\frac{eGFR}{100.0}\right)^{1.02}
V_1 = 46.8 \cdot \frac{IBW}{63.0}
V_2 = 56.1 \cdot \frac{IBW}{63.0}
Q = 6.55 \cdot \left(\frac{IBW}{63.0}\right)^{0.75}
Implementation notes
Two-compartment model developed specifically for North American adults with hematologic cancer (with or without HSCT). Uses Ideal Body Weight (Devine) for all parameters and eGFR (CKD-EPI) for clearance. Additive error model.
