Implementation notes

ICU model (non-CRRT branch). tMCS lowers intercompartmental clearance to exp(-0.66) ≈ 0.52, driven by the ECMO tag (the cohort's tMCS was mostly VA ECMO); CRRT covariate on Vc excluded.
